All athletes must be cleared by the Athletic Office before they can compete in athletics of any kind including tryouts and practices. You must complete the entire registration online.
Please make sure you apply to only one team per season Fall/Winter/Spring. Remember to update your physical annually before the date of expiration. Your physical must be signed by an MD/DO/PA/NP. Within this registration you will find detailed information about Academic Eligibility, the BHS Activities Code, CIF Code of Ethics, Sudden Cardiac Arrest, Concussion Protocol, Heat Illness, the Risk of taking Opioids, and other important athletic information. All forms must be signed by both the parent/guardian and the athlete. Please take time to review all materials and ensure all signatures are complete before submitting these forms.
WE DO NOT ACCEPT HARD COPIES OF ANY OF THE FORMS
EVERYTHING MUST BE TURNED IN TO THE CLEARANCE SITE
